What We Do

Guided by the Holy Spirit, Changed by Christ walks with women reentering the community in Clinton County — peer support, case management, and a place to rebuild.

Overline
Phase One

Nothing else works until the ground is steady. We help people get identification and essential documents, find housing, access food, and connect to treatment. Screening and navigation through the Healthy Alliance Social Care Network opens doors to utilities assistance, benefits, and the referrals that turn an urgent need into a solved one.

Build
Phase Two

With the basics in place, the work turns to what comes next. Counseling, mentoring, and vocational training build skills and confidence. Peer support from people with lived experience means walking alongside someone who has been there. And because a missed ride is a missed appointment, we coordinate transportation as part of the plan.

Sustain
Phase 3

Reentry doesn’t end with a job and an address. Health Home Care Management keeps medical care, behavioral health, benefits, and housing coordinated in one place, and ongoing connection to community resources means the support system holds long after the transition.
